筛查手段各有优劣亟需补足
近年来，随着全球老龄化进程的加速，与其密切相关的神经退行性疾病导致的危害也日益加剧。以我国为例，目前60岁及以上的阿尔茨海默病（AD）患者数量约有1500万，位居世界首位，由此导致了高昂的社会、经济成本——预计到2030年，我国阿尔茨海默病的社会年度总成本将超过3万亿元。
但是，以AD为代表的神经退行性疾病的防治一直未能取得明显的效果。这主要有两方面原因。
首先，神经退行性疾病的病程通常较长，且与年龄高度相关，其早期表现往往被误认为是衰老的正常现象而被忽略。根据统计，85%以上的阿尔茨海默病患者初次就诊时就直接被确诊为中重度阿尔茨海默病，跳过了认知衰退的早期阶段，无力回天。
其次，由于大脑这个由上千亿神经细胞组成的复杂器官与人体的关系尚未被研究透彻，多数神经系统疾病至今缺乏有效的治疗手段。近二十年来，全球先后有大约百余种针对阿尔茨海默病的药物进入测试阶段，但获批可用的药物寥寥无几，且它们的适用范围和症状改善效果比较有限，无法逆转认知损害或显著缓解疾病进展。只有在早期的轻度认知障碍（MCI）阶段加以干预，才可能有效地延缓疾病病程进展。
因此，尽早发现MCI对于防治的意义十分重大，我国近年来也开始加强相关的防治工作。
尤其是2016年发布的“健康中国2030”规划纲要，提出了“加强老年痴呆症等的有效干预”的要求。2020年，国家卫健委据此发布了《探索老年痴呆防治特色服务工作方案》，2022年，国家卫健委等15部门联合印发《“十四五”健康老龄化规划》，并在今年5月发布了《国家卫生健康委办公厅关于开展老年痴呆防治促进行动(2023-2025年)的通知》。
在政策的强力推动下，全国范围的大规模认知障碍筛查工作正迅速拉开帷幕。
目前，认知障碍筛查主要有几种手段，首先是传统的神经心理量表和主观观察，其次是使用核磁共振等影像技术识别脑结构的改变，则是外周血、脑脊液等生物标志物检查。不过，这些方法目前在大规模认知障碍筛查工作中都存在一些明显的局限性。
神经心理量表是临床普遍使用的认知障碍筛查的手段，但其存在几个难点。一方面，量表包含了几百个问题，正常完成需要半个多小时，在筛查阶段几乎没有患者愿意填写。另一方面，量表对于受试者个人文化水平有所要求，若文化水平不足，难以理解问题则会使得量表的客观性受到挑战。
核磁共振等影像技术虽然能够精确地反映脑结构的变化，但到目前为止，依靠影像仍然无法提前识别脑结构没有变化时的认知状况，而且也很难通过细微的结构变化来判断风险。此外，核磁共振的可及性较差，无论设备成本还是检查费用对于基层筛查来说都无法承受。
如脑脊液及外周血这样的生物标志物检测因为可以较早检测出脑功能的变化，近年来快速崛起，但在临床实践中对实施场所有所要求，且存在标本采集困难、价格昂贵等现实问题。
基于这种现状，对于神经功能的表观识别方法以其固有的客观性与便利性，一直被认为是有前景的筛查手段。其中，最受关注的方向就是针对步态和眼动行为的定量分析评价。不过，此前的步态和眼动评价设备在技术上存在一些限制，如繁琐的穿戴过程和复杂的校准流程，因此，在大规模筛查中的有效性和广泛适用性尚未得到充分验证。中南大学湘雅医院神经内科沈璐教授团队此次的研究则证明了这一分析评价方法在早期筛查中的可行性与潜力，是一次巨大的突破。
