重症肺炎患者血清中 sTＲEM-1、纤维蛋白原及
D-二聚体水平变化及临床意义

【摘要】 目的 分析重症肺炎患者血清中可溶性髓系细胞触发受体 1( sTＲEM-1) 、纤维蛋白原( FBG) 及D-二聚体( D-D) 水平变化及临床意义。方法 回顾性分析我院 2015 年 1 月 － 2017 年 7 月收治的 76 例重症肺炎患者资料，按照疾病转归情况分为死亡组( 34 例) 及存活组( 42 例) ，同期各选择 76 例一般肺炎患者及健康者。比较各组血清 sTＲEM-1、FBG、D-D 水平及 PSI 指数变化并分析之间的相关性、和预后的联系。结果重症肺炎组血清 sTＲEM-1、FBG、D-D 水平高于一般肺炎组且高于健康组，重症肺炎组 PSI 指数依次高于一般肺炎组且高于健康组，比较有统计学差异( P ＜ 0. 05) 。死亡组 sTＲEM-1、FBG、D-D 水平及 PSI 指数均高于存活组，比较有统计学差异( P ＜ 0. 05) 。重症肺炎患者血清 sTＲEM-1、FBG、D-D 水平和 PSI 指数均呈正相关，( r分别 = 0. 546、0. 615、0. 497，P ＜ 0. 05) 。经 ＲOC 曲线分析可见，血清 sTＲEM-1、FBG、D-D 联合测定曲线下面积高于单个指标。结论 sTＲEM-1、FBG 及 D-D 在重症肺炎患者血清中高表达，联合检测其水平有利于疾病诊断及预后评估，可作为临床重要的观察指标。【关键词】 重症肺炎; 可溶性髓系细胞触发受体 1; 纤维蛋白原; D-二聚体; 临床意义
Changes ａnd clinical significance of sTＲEM-1，fibrinogen ａnd D-dimer levels in severe pneumonia patients TIAN Xin Department of Ｒespiration，General Hospital of Jilin Oilfield，Songyuan，Jilin 138000，China【Abstract】 Objective To analyze the changes ａnd clinical significance of soluble myeloid cell triggered receptor 1 ( sTＲEM-1) ，fibrinogen ( FBG) ａnd D dimer ( D-D) levels in severe pneumonia patients． Methods 76
patients with severe pneumonia from January 2015 to July 2017 were divided into the death group ( n = 34) ａnd the survival group ( n = 42) according to the disease outcome. During the same period，76 patients with normal pneumonia ａnd healthy people were selected． The levels of serum sTＲEM-1，FBG，D-D ａnd PSI index in each group were compared ａnd analyzed，and the correlation ａnd prognosis was analyzed between each group． Ｒesults The levels of serum FBG，sTＲEM-1，D-D ａnd PSI index were higher in the severe pneumonia group than in the general pneumonia group ａnd the healthy group ( P ＜ 0. 05) ． The levels of sTＲEM-1，FBG，D-D ａnd PSI index in the death group were higher than those in the survival group ( P ＜ 0. 05) ． Serum sTＲEM-1，FBG，D-D ａnd PSI index was positively correlated in severe pneumonia patients，( r = 0. 546，0. 615，0. 497 respectively，P ＜ 0. 05) ． According to the ＲOC curve analysis，the area of the serum sTＲEM-1，FBG，D-D ａnd D-D was higher than the single index． ConclusionsTＲEM-1，FBG ａnd D-D are highly expressed in patients with severe pneumonia，and the joint detection is conducive to disease diagnosis ａnd prognosis assessment，which can be used as an important clinical observation indicator．
